Employee BenefitsEmployee Assistance Program (EAP - Kaweah Delta) All Tulare County Office of Education (TCOE) employees (and their family members including spouses, birth, adopted and step-children through age 19, or age 24 if qualified as a dependent), regardless of working part-time or full-time, are eligible for this free confidential, professional service which is available for those times when your personal problems seem to be taking control of your life and job performance. If needed, EAP may suggest additional resources. EAP provides problem evaluation, referral, and short-term intervention services for help in dealing with issues such as: stress, anxiety, and depression; divorce and separation; conflict in the workplace; care of elderly parents; alcohol or drug concerns; parent-child issues; family/relationship concerns; grief and loss; financial problems; life change; illness and disability; and domestic abuse. More information Employee Assistance Program (EAP - PacificCare Behavioral Health) TCOE employees working full-time (and their eligible dependents) are provided with the Employee Assistance Program by Pacificare Behavioral Health under their SISC III Health Insurance Coverage. This EAP is a free, confidential brief counseling and referral service designed to help you and anyone living in your household to resolve personal problems that may be interfering with your work or home life. To request an appointment, call 1-800-999-9585. ***Note: It may be possible to coordinate the services that both EAPs provide to get the maximum benefit out of these services. When you call to inquire about services, let them know that you have two EAP providers and see if there is a service provider working through both agencies. More information Basic Life / Accidental Death & Dismemberment Insurance All TCOE employees, regardless of working part-time or full-time, are eligible for term life insurance through Prudential in the amount of $50,000. This coverage is terminated on the last day of the month in which the employee resigns, retires, or is terminated, however, it may be continued at the individual's expense if converted to an individual policy. A $1,000 Accidental Death & Dismemberment insurance policy is also included in your health benefits package through SISC. Enrollment Form Delta Dental TCOE employees working full-time (and their eligible dependents) are provided with dental insurance coverage under Delta Dental Premier. Annual maximum benefits renew every January. It is advantageous to go to a participating provider. More information If you are thinking about having extensive dental work, Delta Dental will provide a predetermination to its enrollees free of charge. More information Delta Dental Online Delta Dental has developed a website where an enrollee may check benefits and eligibility; print a customized ID card; find a nearby dentist or check to see if their dentist is a Delta dentist; get answers to frequently asked questions, etc. The Delta Dental Online Customer Service number for northern California is 1-800-765-6003; the southern California number is 1-800-422-4234. If you choose to see a participating provider, the co-pay is $10 per office visit. If you choose to see a non-participating provider, the cost will be greater. More information Prescription Services TCOE employees working full-time (and their eligible dependents) are provided with prescription coverage which is administered by American Health Care. Their telephone number is 1-800-872-8276. Prescriptions may be filled by using the mail-in services provided by DrugSource, Inc. Their telephone number is 1-800-854-8764 and their website is www.drugsourceinc.com. Order Form Health Insurance Coverage TCOE employees working full-time (and their eligible dependents) are provided with health care coverage under Self-Insured Schools of California - SISC III. Their website is www.kern.org/SISC. Foundation for Medical Care is the medical insurance administrator for physicians; Anthem Blue Cross is the carrier. To contact the Foundation for Medical Care regarding coverage and claims, you may call 559-734-1321, or 1-800-668-5502. To order additional insurance cards, you may contact the TCOE Human Resources Department at 559-733-6329, or you may call SISC III at 1-800-972-1727. SISC requires the following documentation to be submitted with all enrollments:
Health Services Providers To access a list of Anthem Blue Cross health care providers, you may go to the Anthem Blue Cross website. You will need to register as a user the first time you visit this website. The link is www.anthem.com/ca/welcome/. It is advantageous to go to a participating provider. Employees are able to go online to print a temporary medical insurance card to be used until they receive their card which is issued by SISC. If traveling out of the country and the need for medical care arises, you can call 1-800-810-2583 and get information regarding providers in the area that you are in that will accept our card. Benefits begin on the 31st day of Total Disability due to a covered accident or illness. The Plan pays a monthly disability benefit of 60% of your Monthly Compensation not to exceed: (1) a maximum monthly Disability Benefit of $3,600; (2) a maximum covered Monthly Compensation of $6,000.00; and (3) the amount for which premium is being paid.
